Commonwealth Care Alliance Massachusetts (CCA) Medicare Preferred (PPO) and CCA Medicare Value (PPO) are health plans with a Medicare contract. CCA Senior Care Options (HMO D-SNP) is a health plan that contracts with both Medicare and the Commonwealth of Massachusetts Medicaid program to provide benefits of both programs to enrollees. Enrollment in the plans depends on contract renewal.
